  10. What is the progress and/or setbacks relative to these goals?

  11. What benefits are derived from the project’s implementation and who does it serve?  Name specific municipalities served by project.

  1. What is the timeline for completion of the project?

  2. Describe results of evaluation of project.

  3. Describe project’s impact.

  4. Did project cause adverse impacts elsewhere?

  5. What would you have done differently if you had the chance?

  6. What would you do the same?

  7. What are your specific plans, if any, for continuing the work started with this project?

  1. How were grant funds spent.  Provide financial report with final report. (use attached form)

  2. What did you learn from the project?
